Telemonitoring is an innovative service for patients at risk for hospitalization, newly diagnosed with a chronic disease or those who would benefit from daily evaluation by a licensed nurse.
The goals of Telemonitoring are:
Each day, the Telemonitoring unit placed in patients’ homes guides them through a simple, three-minute process with step-by-step instructions. People are able to record their weight, blood pressure, pulse, temperature, oxygen saturation, and blood glucose. The data is then securely transmitted to the Aspirus At Home office where a nurse reviews the information and tracks for significant change.
Based upon criteria established by the physician in advance, the nurse responds with telephone intervention as the doctor prescribed that includes further assessment, teaching and follow-up communication with the patient’s physician.
Services offered by Aspirus At Home are covered by a variety of sources including: private pay, private insurance, Medicare, Medicaid, and other sources. A staff person from Aspirus At Home will work with you and your family to evaluate all payment options prior to receiving care.

At Aspirus Help At Home, we recognize that many people want the benefits of assistance at home. That’s where we come in – to bring affordable home care assistance right to your door so that you can remain as independent as possible for as long as possible.
We will meet with you to help you determine the services that best meet your needs and then provide you a no-obligation quote.
